Can Advanced Age be a Risk Factor for Intracerebral Hemorrhage after Deep Brain Stimulation Surgery?

Abstract
Purpose
Deep brain stimulation (DBS) is a surgical option for movement disorders including idiopathic dystonia, Parkinson's disease, and essential tremor. However, intracerebral hemorrhage (ICH) can occur after this surgical treatment and may have serious consequences for patients. Therefore, we investigated the risk factors of ICH after DBS.
Methods
From 2011 to 2018, a total of 110 patients who underwent DBS at the Catholic University of Korea Incheon St. Mary's Hospital were retrospectively investigated. Among the risk factors mentioned in many previous reports, we examined the effects of age, sex, hypertension, associated comorbidities, and DBS targets on the bleeding risk.
Results
Subdural hemorrhage occurred in 2 of 110 patients (1.8 %) and ICH in 6 of 110 patients (5.5 %). There were no cases of epidural or intraventricular hemorrhage. Our study showed that age is a statistically significant cause of postoperative ICH, but not sex, associated comorbidities, hypertension and DBS targets.
Conclusions
The study results indicate that elderly patients have a high risk of ICH after DBS. This suggests that DBS surgery, especially targeting elderly patients with degenerative brain changes such as atrophy and cerebral small vessel disease should be performed with caution.
